Please review before scheduling:

  1. Free initial phone consultation:  After receiving your appointment request, I will call you for a free 30 minute phone consultation.  This is a great opportunity to discuss what's happening for you now, answer any questions you have, and give you a sense of my style and approach to treatment.  If you prefer to consult with me before booking an appointment, please find an appointment time following the steps below then send me a message, including the time you'd like to be seen, and I'll be in touch within the same business day.     

  2. When requesting an appointment, choose a time that works for you on a weekly basis.   We can change our meeting time down the road, availability permitting, but it's the best place to start.

  3. "Private Pay" or "Insurance"You'll be asked which option you prefer when requesting an appointment.  Questions?  Please visit Fees & Insurance FAQs for more information. 
     

  4. Acceptable forms of payment:  A credit card is required to book your first appointment but you won't be charged until after services are rendered.  You may use other forms of payment when we meet including cash, check, or credit/debit cards (including FSA/HSA cards).

  5. Cancellation policy for initial appointments:  Please provide 24-hours notice if you need to cancel your first appointment to avoid being charged.
